Positive Kids confidentiality form

The services we offer are confidential unless you give us consent in writing to communicate with someone on your behalf. There are exceptional circumstances where strict confidentiality cannot be maintained because of ethical or legal requirements.

Limits of confidentiality may include

• Clinical records being subpoenaed by court.
• Suspected child abuse or neglect.
• Preventing harm to yourself or someone else.

Child therapy services

The service provided is intended to address your presenting needs. You and your counsellor, coach, or facilitator will discuss your situation, establish goals, and review the number of sessions anticipated. If another professional is required, Positive Kids will make every effort to assist with a referral.

Cancellation of appointments

Please provide at least 24 hours notice if you are unable to keep an appointment. A missed appointment may be charged where no notice was provided.
